Home and Community Programs
Bancroft offers Home and Community-Based Cognitive Rehabilitation Therapy to maximize functional independence and community re-integration. We offer individualized programs originating from a person’s home, job site, and other community locations. Individualized assessment and treatment, case management, family education and training, are provided to adults with acquired brain injuries and other neurological conditions. These services can be combined with Outpatient Rehabilitation treatment.

Treatment Benefits

1. Extend rehabilitation gains to home, work and school;
2. Develop and refine compensatory strategies in real life settings;
3. Increase opportunities for successful transition to home following hospitalization
4. Assist individuals and families with identifying and accessing allied health care and other resources in their local communities;
5. Continue providing structure and therapeutic support post-discharge from residential treatment;
6. Support persons in the least restrictive environment; and
7. Provide cost-effective care

Admissions Criteria

1. Medically and psychologically stable
2. Free from active substance abuse
3. Willing to participate in rehabilitation
4. 18 years of age and older

Funders
Bancroft Brain Injury Home and Community Services are funded by private health insurance, automobile insurance, Workers’ Compensation, the New Jersey TBI Medicaid Waiver, the New Jersey Traumatic Brain Injury Fund, and the New Jersey Division of Vocational Rehabilitation.
